Title: Health Care Home RN Coordinator (Medical Home) Location: Main Campus Date Posted: August 26, 2010 Work Schedule/hours: 40 hours/week Responsibilities: This position is part of the care team acting as access liaison, resource, educator, coach, advocate, and partner for patients enrolled in a Health Care Home (HCH) Practice.
