Title: Provider Assistant - Dr. Hipp
Location: Specialty Clinic

Work Schedule/hours:  17 hours/week (including weekend rotation)

Responsibilities: Patient care responsibilities include collecting patient history and vital signs, giving injections, obtaining lab reports, hospital notes and referral information, returning patient calls, assisting the provider with procedures as needed. Clerical responsibilities include scheduling appointments and completing forms as needed.

Qualifications: Current CMA or LPN license and CPR certification required. Knowledge of basic patient care principles and protocols, policy and procedures related to infection control, safety and patient confidentiality; skill in appropriate rooming and assistance techniques. Ability to provide quality customer service to patients in a fast paced clinic setting in calm and caring manner that fosters patient health.

Title: QI Data Specialist
Location: Main Campus - Administration

Work Schedule/hours:  40 hours/week EXEMPT position

Responsibilities: This position will support our efforts to improve clinic operations and patient satisfaction by providing data support for multiple clinic measures. Responsibilities include data collection and retrieval, organizing and analyzing the data, as well as presenting the information to individuals and committees using graphs, charts and other presentation tools.

Qualifications:   Bachelor’s degree plus experience with Crystal Report Writing is required. A minimum of 2 years experience with designing databases, including an understanding of query fundamentals and relational databases is preferred. Prefer experience in the fields of medicine and statistics. The successful candidate will have excellent verbal and written communication skills, the ability to articulate data findings to internal and external customers, and proven ability to work independently, prioritize, stay organized and perform detail-oriented work.

Responsibilities: Plans, evaluates, administers and adjusts treatment plans in coordination with clinic staff; educates patients and family in proper care and use of various support and exercise programs, evaluates; charts and reports on patient’s progress; completes accurate and timely documentation and maintains patient records, charts and discharge summaries as required.

Qualifications: Advanced degree in Physical Therapy. Competence in physical therapy evaluation and treatment. Strong interest in clinical excellence and professional development.
