Responsibilities: This position is part of the care team acting as access liaison, resource, educator, coach, advocate, and partner for patients enrolled in a Health Care Home (HCH) Practice. Working together with patients (and their families) who have or are at risk for chronic or complex conditions, the Care Coordinator provides information about Health Care Home services, helps patients and families implement and actualize individualized care plans and plan for productive provider visits.
