Stillwater Medical Group maintains a single medical record for each of our patients. Your record is kept at the clinic where you are most frequently seen.
Your record is the property of Stillwater Medical Group and it is protected by law. If you would like to have copies of your records sent to another physician or health care facility, insurance company, or attorney, please send a signed authorization to Stillwater Medical Group. Authorization forms are also available at each clinic location. There may be a fee for these copies. For more information, contact the medical records department at your primary clinic location.
